2023-2024 Physical & Concussion Forms

Before the student-athlete is allowed to practice, they must have the following in the high school office:

1. A current physical is completed and signed by a physician, the student-athlete, and a parent/guardian.
2.  A concussion form signed by the student-athlete and a parent/guardian.
